কোন কোন ক্ষেত্রে Fetal Echocardiography করা উচিত?

Fetal Echocardiography মূলত গর্ভের শিশুর হৃদরোগ আগেভাগে শনাক্ত করার জন্য করা হয়, বিশেষ করে নিম্নোক্ত অবস্থায়:


🔴 মাতৃজনিত কারণ (Maternal Indications)

✔ বয়স ≥ ৩০ বছর
✔ ডায়াবেটিস (pre-gestational বা uncontrolled GDM)
✔ উচ্চ রক্তচাপ (Hypertension)
✔ Rubella / TORCH infection
✔ Autoimmune disease (SLE, SSA/SSB positive)
✔ Phenylketonuria (PKU)
✔ Teratogenic drug exposure
(antiepileptic, lithium, retinoic acid)
✔ IVF / Assisted pregnancy


🔴 ভ্রূণজনিত কারণ (Fetal Indications)

✔ Increased Nuchal Translucency (NT)
✔ Abnormal anomaly scan
✔ Suspected congenital heart disease
✔ Fetal arrhythmia
✔ Hydrops fetalis
✔ Single umbilical artery
✔ Extracardiac anomalies


🔴 পারিবারিক ইতিহাস (Family History)

✔ পূর্বে জন্মগত হৃদরোগযুক্ত বেবি
✔ First-degree relative এ CHD


🔴 আল্ট্রাসাউন্ডে অস্বাভাবিকতা

✔ Abnormal 4-chamber view
✔ Outflow tract abnormality
✔ Cardiomegaly
✔ Pericardial effusion


🟡 অন্যান্য বিবেচ্য কারণ

✔ Twin pregnancy (বিশেষ করে monochorionic)
✔ Unexplained polyhydramnios
✔ IUGR with suspected anomaly


⚡ জরুরি রেফারাল

✔ Suspected major CHD
✔ Persistent fetal arrhythmia
✔ Hydrops fetalis

Indications for Fetal Echocardiography

Fetal echocardiography is recommended for early detection of congenital heart disease, especially in the following conditions:


🔴 Maternal Indications

✔ Maternal age ≥ 30 years
✔ Diabetes mellitus
✔ Hypertension
✔ Rubella / TORCH infection
✔ Autoimmune disorders (SLE, SSA/SSB)
✔ Phenylketonuria
✔ Teratogenic drug exposure
✔ IVF / Assisted reproduction


🔴 Fetal Indications

✔ Increased Nuchal Translucency
✔ Abnormal anomaly scan
✔ Suspected CHD
✔ Fetal arrhythmia
✔ Hydrops fetalis
✔ Single umbilical artery
✔ Extracardiac anomalies


🔴 Family History

✔ Previous baby with CHD
✔ First-degree relative with CHD


🔴 Ultrasound Findings

✔ Abnormal 4-chamber view
✔ Outflow tract abnormality
✔ Cardiomegaly
✔ Pericardial effusion


🟡 Additional Considerations

✔ Twin pregnancy (especially monochorionic)
✔ Polyhydramnios
✔ IUGR with anomaly


⚡ Urgent Referral

✔ Suspected major CHD
✔ Persistent arrhythmia
✔ Hydrops fetalis
